I understand that no doctor, nurse or administrative personnel has prior knowledge as to whether I would, or would not have adverse effects due to the fact that each individual has a different biological and chemical make-up. I am participating in this program at my own choice, at my own expense and my own liability and I assume all responsibility for my use of Celebrex. I fully understand that Access RX holds me responsible to have an annual complete history, physical exam, and lab tests to ensure that I have no diseases) present before entering into the Celebrex program.
